I Shri. Shreeram Kanu Khadapkar an adult of Bombay, aged about 54 years, residing at 4/6, Poly Demelo Chawl, Near Damle Colony, Bhandup (E), Mumbai – 400 042, do hereby state and declare on solemn affirmation as under:-
I say that I am father of Gokula Shreeram Khadapkar who is also known as Deepika shreeram Khadapkar. My daughter Gokula @ Deepika is a student of 10th Std. of Indian Education Society of Bhandup, and her name is entered on the school registered as Gokula but she is known at home as Deepika, that being her pet name.
I say that she has won the prize of Mumbai Chaupher Jumbo Shabdkode No. 69 (B) and she is entitled for the same.
I am making this affidavit to produce before the authorities of Mumbai Chaupher Jumbo Shabdkode No. 69 (B). I am making this Affidavit being Legal Guardian of Deepika S. Khadapkar since she is a minor aged about 15 yrs.
I say that in the abovementioned Mumbai Chaupher Jumbo Shabdkode No. 69 (B) she has entered her participation in the name of Deepika S. Khadapkar instead of Gokula S. Khadapkar.
I say that Deepika S. Khadapkar and Gokula S. Khadapkar are one and the same person and hence entitled for the prize won in the Mumbai Chaupher Jumbo Shabdkode No. 69 (B).
I say that what is stated hereinabove is true and correct.
